Referral Process
What information is required to finalize a referral?
- Consumer Face Sheet: This document contains essential details for effectively reaching out to the consumer and their family. Ensuring the accuracy of this information is crucial for successful initial contact.
- Purchase of Service: This document helps the agency review the allocated hours for the consumer and establish a staffing schedule.
- IPP/IEP: This document provides insight into the consumer’s needs and the goals that have been established.
We require the following details to reach out to the consumer and their family effectively. Initially, we aim to address any questions they may have about our agency, helping them decide if we are the right choice for their needs.
Once they confirm their decision, we will gather further information needed for staffing purposes. An In Home Representative regularly updates the Service Coordinator about the case’s progress.
